HeartPath was built by nurses, for the families who need one most. We provide compassionate, high quality in-home care for children with medical needs, supporting their health, comfort and independence, and giving their parents help they can count on.

You have been carrying this on your own, and carrying it well. We are not here to take over your home. We are here to stand beside you in it, so the hardest parts stop falling to you alone.

Physician, case manager, discharge planner or early intervention coordinator? GAPP runs from birth to 21, so a child can be on Babies Can't Wait and GAPP at the same time. We screen for potential eligibility and handle the GAPP paperwork.

Your child is not a shift on a schedule. The nurse in your home keeps them safe and keeps them a kid: part of the noise, the routine, the family. That is the difference between a shift being covered and a child being cared for, and it only works when the right person walks through your door.

So the match comes first

So we start with fit: the right clinical experience for your child's condition, and the right temperament for your household. When the match is right they come back, and the person who knows your child's baseline is the one who notices the day it changes.

Ventilator dependence, tracheostomy, central lines and TPN, feeding tubes, seizure and neurological disorders. If you have been told your child is too complex for home care, read us the orders. We will give you a straight answer, and if we are not the right fit we will say so.

Every nurse and CNA we place is background checked and fingerprinted before they ever meet your child. Then they sit down with Andrea, our parent liaison. She is a mother raising a medically fragile child of her own, with a background in behavioral health. She has sat exactly where you are sitting, and she is listening for what a license cannot show you: whether their heart is really in it.

A Medicaid-funded program that pays for care at home instead of in a hospital, for children under 21 who are medically fragile. HeartPath is an approved provider.

How families usually qualify

  • Have a medical condition requiring ongoing skilled care
  • Have doctor-ordered services

Income too high for Medicaid? The Katie Beckett Waiver lets a child qualify on their own medical needs instead of parental income. Your family files that one, not us. Once you are approved, we can usually take it from there. See if it is your path.

What it costs, and how long it takes

Cost
No out-of-pocket cost for eligible families. Care is covered by Georgia Medicaid.
Other coverage
Private pay is available. A child on PeachCare is not eligible for GAPP nursing, and the two run on separate tracks. Here is the sequence that does work. On CareSource, Amerigroup or Peach State? That is regular Medicaid through a care management plan, and your child can still get GAPP. We verify the coverage and explain the change that has to happen before GAPP services begin.

Georgia Medicaid reviews and approves every case. HeartPath cannot approve or deny anyone.

How GAPP works

Three ways families use the program

GAPP covers two different kinds of hours. Skilled hours are the ones that need a nurse. Personal care support hours are the everyday hands-on help with bathing, dressing, feeding and transfers, and those do not need a nurse. Your child's approved plan of care decides the mix.

1 Skilled hours

A licensed nurse in your home

An RN or LPN delivers your child's skilled care one to one, on the schedule the orders call for, from a few shifts a week to around-the-clock coverage.

A CNA for the hours that do not need a nurse

Not every approved hour requires a nurse. For the everyday hands-on help, we send a certified nursing assistant, with the same matching and the same oversight from our nursing team.

Frequently asked questions

GAPP is the Georgia Pediatric Program, a Medicaid-funded healthcare program that provides in-home medical care for children under 21 who are medically fragile. HeartPath Home Care is an approved GAPP provider.

There is no fixed list of diagnoses. Approval is based on how much hands-on care a child needs day to day. Children who qualify typically depend on a tracheostomy, a ventilator, a feeding tube, oxygen or seizure care, or need significant help with lifting and transfers. Cerebral palsy, spina bifida, neurological disorders and other complex conditions may qualify based on a child’s skilled-care needs.

Georgia Medicaid decides. Every case is submitted for review and approved based on the child's medical needs and how much care is involved. HeartPath cannot approve or deny anyone. What we can do is tell you where you stand and help you get the paperwork moving.

There is no out-of-pocket cost for eligible families. Care is covered by Georgia Medicaid.

Yes. The Katie Beckett Waiver lets a child qualify on their own medical needs rather than parental income. That application is one your family files with the state, not something HeartPath files for you. Once your child is approved under Katie Beckett, we handle the GAPP paperwork from there.

In some GAPP cases a family member can be hired and paid as an employee to provide their child's approved personal care hours. This pays for personal care support, not skilled nursing, and whether it is approved depends on your child's age, care needs and approved plan of care. In our experience it is less common for very young children, because reviewers expect care before that age to be mostly medical. That is a pattern, not a published rule. Skilled nursing has no such age pattern. Foster parents are not eligible. Whether it applies to your family depends on the approved plan of care. Ask us and we will walk you through it.

That is the whole point of how we staff. We match nurses to their client and work to keep the same nurse with the same family, because the nurse who knows your child's baseline is the one who spots the day it changes.

Yes, your child can attend school. What GAPP will not do is pay for hours delivered at school. Program rules exclude the school setting, because that overlaps with what the school itself is responsible for under an IEP. So GAPP hours are scheduled around school: mornings, evenings, overnights, weekends, holidays and summer. If your child has an IEP, the current one is part of the paperwork, so keep a copy handy.

GAPP does not pay for hours while your child is an inpatient, and it does not pay for care delivered outside Georgia. Hours pause and pick back up when your child is home. Tell us as soon as you know about an admission or a trip, so nothing is billed wrongly and your nurse's schedule is protected while you are away.

GAPP is a Medicaid service, not a payment to your family, so the nursing hours themselves are not income and do not count against SSI. One thing to watch: if you are hired and paid as a family caregiver, that is wages, and earned income in the household can affect a child's SSI. It is worth a call to your SSI caseworker before you start, so there are no surprises. We would rather you know that up front than find out on a benefits letter.

There is no long-term contract, and you are not stuck with us. GAPP has a formal transfer process, and families change providers when a placement is not working. If you are with another agency now and want to move, we can walk you through the transfer. If you are with us and it is not working, we would rather help you move than have your child go without the right fit.

Hours come from the plan of treatment your child's physician signs, and Georgia Medicaid authorizes against that. So more hours start with the doctor. If your child's condition changes, tell us and tell the physician, and we will help get updated documentation submitted. Be careful with any agency that promises more hours quickly. Nobody can promise that, because nobody but Medicaid decides it.
